At the Centre for Development Disability Health Victoria (CDDHV), our goal is to improve health outcomes for people with developmental disability.

The Appointment System

Note: The CDDHV does not provide ongoing medical or psychiatric management to the patients it assesses.

Please note that the CDDHV clinics are for clients aged 18 years and over.

To obtain an appointment to see a CDDHV clinician, a Pre-Appointment Questionnaire (PAQ) needs to be completed (including section B for the referring doctor to complete) and returned to the CDDHV. The PAQ will then be reviewed by the CDDHV clinicians and if an appointment is warranted appointment details and information will be sent out to the client.

If an appointment is not offered, this will be discussed by phone with the referring doctor and the referring support person and alternative approaches to the problem will be suggested.

Follow-up or review appointments can be made at the end of the consultation or by phone. The doctor will indicate at the consultation or in the report whether and when a further appointment is required.

If a client has been seen at the CDDHV in the past and wishes to be seen again, a new PAQ may need to be submitted as information may have changed. This is particularly so if it is over two years since the client was last seen and the file was closed after the previous assessment.
